New Vendor Registration
All potential vendors are required to submit a vendor qualification form to the City and receive approval prior to doing business with us. Provide basic business information through the Vendor Qualification Form. You can also upload your W-9 in the form. If applicable, submit a separate copy of your business license and/or professional liability insurance certificate.

Complete the form to be verified as an approved vendor and upload your W-9. This form is required to be completed prior to receipt of payment for any goods or services provided to Eagle Mountain City.

Vendor Approval
Eagle Mountain City will review applications and references will be checked. Potential vendors may be contacted for additional information or clarification. Incomplete applications will not be approved. Once the vendor has been verified and approved, the vendor will receive confirmation. All applications will be evaluated using standard criteria.
